Nelson Bays Primary Health Kaiatawhai service supports Māori, Pacific, refugee, migrant and all other families/whānau who have a community service card to connect with primary health care providers.  We help remove barriers to health care and support families to understand and navigate the health environment.

The Kaiatawhai service is a navigation and case-management service.  We can visit whānau/families in their home for consults and assessments to understand the issues, barriers and/or challenges to accessing primary health care services.  The service will support whānau to navigate the primary health care pathways so they can access the right service in a timely fashion.
